    Music Therapy Assessment

    Preparing for Your Initial Visit

    Once your assessment is scheduled, it’s time to fill out paperwork. (We promise it gets better after this part!) It is very important that we receive your completed Face Sheet and Scheduler form no less than 48 hours before your first appointment. If we do not receive completed paperwork within 48 hours of your scheduled appointment, your assessment may be rescheduled.

    Other Paperwork

    We also request that you gather the most current assessment reports and progress notes from other providers on the team including Speech Therapy, Occupational Therapy, Physical Therapy, Psychotherapy, and other related disciplines integral to the treatment of the client. Please bring copies with you to the initial visit as your therapist will review these prior to completing the Music Therapy Treatment Plan.

    What to Expect During the Assessment

    1) Check-in: Brief conversation with you, the client, (and/or parent/guardian/spouse as appropriate) to confirm receipt of all completed paperwork and review information received during the intake process.

    2) Active Musicing: You will be presented with a wide variety of music-based experiences to determine strengths and needs, establish a baseline of current abilities, and assess your general response to music as well as explore music preferences. These therapeutic music experiences may include playing music, moving to music, singing, or listening to music. We often find that the presence of a parent/guardian/spouse impacts the client’s responses and therefore may skew the assessment data. To avoid this, the therapist and client will work one on one unless parent/guardian/spouse participation is deemed necessary or appropriate by the assessing music therapist.

    3) Follow-up: At the end of the assessment session, your therapist will provide a brief verbal synopsis of the session, answer any questions which may have arisen, and confirm your follow-up appointment.

    Your Music Therapy Treatment Plan

    After your appointment, the therapist will review your assessment data and compare with the information from the other reports provided as well as information received during the intake process. All of this data will be referenced to create the Music Therapy Treatment Plan. This plan will serve as a road map for your music therapy services and will include proposed goals and objectives as well as justification directly related to your assessment findings. Your therapists will go over your Music Therapy Treatment Plan at your follow-up appointment which will also be your first treatment session.
